Annual Recertification
AMHA Conducts Annual / Triennial Recertifications
AMHA may process your annual recertification by mail and/or online. The annual recertification is not considered complete until all forms are completed and signed, and all documents that are needed have been received by the Recertification Department.

Online
Complete the online form (see below). Attach any documents that are needed. You must also sign the required release forms (see below)
B
By Mail
Completely fill out and sign the forms mailed to you. Mail back along with any documents that have been requested.
